San Antonio's patient population is defined by two things civilian metros rarely combine. The first is the military: active-duty service members, their families, and a large retired veteran population, served by a military health system and a VA network that both hire NPs directly. The second is South Texas — a predominantly Hispanic region with a heavy chronic disease burden, a substantial uninsured share in a state that has not expanded Medicaid, and rural counties south and west of the city that refer inward for anything specialized. Bilingual Spanish capability is broadly valued across both.

   The city's hospital district: academic, specialty, children's, and private hospital campuses concentrated in one northwest quadrant. The densest employer cluster in the metro.

2. 02Downtown and the near East Side

   The county public system's downtown footprint and the community clinics around it — the metro's safety-net core and its broadest patient mix.

3. 03Fort Sam Houston and the northeast

   Brooke Army Medical Center and the Joint Base San Antonio installations. Federal and contractor NP roles with federal scope rules, federal pay scales, and a longer hiring process.

4. 04Stone Oak and far north Bexar County

   The affluent northern growth belt, where commercially insured suburban outpatient, specialty, and urgent care capacity keeps expanding.

5. 05South Bexar and the surrounding South Texas counties

   Rural and border-adjacent communities with persistent shortage designations, where community clinics and rural health sites recruit NPs for broad-scope primary care.
