The Home Health role at a glance

Home health nurse practitioners bring primary and transitional care to patients where they live — homebound older adults, medically complex patients recently discharged from the hospital, and house-call panels run by value-based care groups.

The workday is territory-based: a route of home visits with independent assessment, medication reconciliation, and care-plan coordination with families, home-health agencies, and collaborating physicians. Annual wellness visits and chronic-care programs make up much of the volume.

Requirements & certification

  • Graduate NP degree with national certification through AANP or ANCC (family or adult-gerontology track, matched to the patient population)
  • Active state APRN licensure and DEA registration
  • Comfort practicing independently — home visits mean assessing and deciding without an on-site team
  • A valid driver’s license and willingness to cover a defined visit territory
